Non-Opioid Directive Form
The Michigan Department of Health and Human Services has developed a non-opioid directive form, which allows patients to voluntarily inform their healthcare providers that they do not wish to receive opioids, while the directive is in place. Individuals (or legal guardians) who wish to execute such a directive may download and complete the form and give a copy to all their healthcare professionals, including dental providers.
